Primary Care Outreach Program

The Primary Care Outreach to Seniors helps link isolated and/or vulnerable seniors living at home to access the care and resources required to live safely in their own home. North Lanark CHC partners with South East Ottawa CHC to deliver the PCO program to seniors in our community.

Our PCO team consists of a registered nurse and a community health worker. They work in collaboration with other health care professionals, geriatric partners, community agencies and the Community Care Access Centre to help vulnerable seniors live safely in their homes, reducing the number of emergency room visits, hospital readmissions and isolation.
