What would make you and your family healthier? What would make our community healthier? Please tell us what you think!

Every 5 years, the North Lanark Community Health Centre conducts  a community health survey to help guide the development of our primary health care and health promotion programs and services. Special emphasis in this survey is given to developing a greater understanding about needs in our community related to poverty, mental health, addictions and overall health services.

The survey will take 10-15 minutes to complete. Responses are being collected until August 5, 2011
