Primary Care Asthma Program (PCAP)
The Primary Care Asthma Program at the North Lanark CHC began as a research project to test a best practice for asthma care. This Asthma Program was based on best practice and consisted of assessment, breathing tests, education and self management and follow up.
The research study lasted 4 years and was proven to decrease Emergency visits by 50%. It also decreased school and work absenteeism, the number of night time awakenings due to asthma and helped our clients be active partners in their own care.
In 2006 this program transitioned into a MOH funded program which we continue today. The Asthma Educator / Respiratory Therapist now sees all clients of any age and their families with the goal of taking better control of their asthma and recognizing when their asthma is worsening.
The Asthma Educator / Respiratory Therapist provides education, testing and a written action-plan for self-management for clients with asthma. Registered clients with asthma are referred to the Asthma Educator / Respiratory Therapist by their physician. Please talk to your doctor if you are interested in this program
